Caregiver responsibilities:
- Daily personal care such as bathing, grocery shopping, oral care, cooking meals, etc.
- Assist and provide help with the daily living activities per the Plan of Care
- Track your patient's condition and/or report to the agency if there is something unusual
- Assist patient with daily meals
- Basic housekeeping functions
- Accompany patient to doctor's appointments
- Assist with essential errands
- Communicate with Summit regarding any issue that your patient might have
- Follow agency's policies
Caregiver requirements:
- New York Home Health Aide Certificate or Personal Care Assistant Certificate
- Government issued
- Social Security Card
- Work permit, if needed
- 2 Reference letters
- Recent physical
- Quantiferon
- Rubella and Rubeola lab work
- Chain of Custody Drug Screen, no older than 6 months
We would rather have your Flu and/or Covid vaccine records but it's no longer a requirement.
