A licensed home care agency looking for one person to read every chart properly before it closes.

People Care has served New York patients and families for 50 years
. We employ more than 4,300 caregivers across New York City and Long Island. This seat has one job, and it is the one nobody notices until an auditor does. Our last internal audit read 94 patient files and found two fully compliant. You are the reason that number changes.

• Every chart, before it closes. Not a sample. Once a chart closes a gap stops being a correction and becomes a remediation — so the review happens first.
• Documentation that matches the care. Confirming what is recorded matches what was authorized and what was actually performed, and that the clinical notes substantiate the visit.
• Compliance against LHCSA and DOH standards. Signatures, dates, orders, assessment elements — checked against current regulatory and payer requirements.
• Findings raised while they are fixable. Incomplete charts go back to the clinician with what is missing stated plainly, and you track them until they close.
• Patterns, not just instances. When the same gap keeps appearing, you say so — it is a training problem, and fixing it once beats fixing it fifty times.

• You are someone who cares deeply about patients and the caregivers who serve them.
• You are transparent — you surface a coverage problem early rather than hoping it resolves itself.
• You go beyond what the task asks when a patient's care depends on it.
• You row together with coordination, intake and clinical rather than working around them.

2+ years of experience coordinating home health or personal care services

Knowledge of Medicaid, managed care, and home care documentation standards

HHA Exchange or equivalent home care platform experience

• Audit readiness. A review record good enough to answer a DOH, Omega or payer review from evidence, on demand.
• Escalation. When a documentation gap looks like a care problem rather than a clerical one, it goes to clinical leadership immediately.
• Current NYS LPN license in good standing. This is non-negotiable.
• Home care judgment. Experience in home care, case management, or scheduling — LHCSA setting preferred.
• Care coordination fluency. Familiarity with LHCSA operations, care coordination workflows and service authorization.
• Organization under pressure. Multiple priorities, a moving schedule, and a phone that keeps ringing — handled without losing the thread.
• Communication. Professional and clear with patients, families, nurses and physicians alike.
• Systems comfort. EMR and scheduling platforms. Strong general computer skills.
• Presence. On-site in our Brooklyn office.
• Competitive Pay. $30 to $35 per hour, based on experience.
