Position: Recreation Leader
Location: Brooklyn, NY
Schedule: Part-Time | Monday–Friday | 4:00 PM – 8:00 PM
Salary: $24.63 per hour

Position Summary

Spring Creek Rehabilitation & Nursing Care Center is seeking a compassionate and energetic Recreation Leader to join our team. This role is essential in enhancing the quality of life for our residents by planning and facilitating engaging recreational activities that support their physical, emotional, and social well-being.

Key Responsibilities
  • Assist in planning, organizing, implementing, and evaluating recreational programs for residents

  • Observe and document resident attendance, mood, behavior, and level of participation

  • Develop and implement individualized and group activity plans based on resident needs and interests

  • Collaborate with interdisciplinary teams on care plans, assessments, and discharge planning

  • Engage residents and families in activity planning and goal setting

  • Conduct one-on-one and group activities, including games, crafts, social events, and reality orientation sessions

  • Encourage participation to promote self-esteem, independence, and social interaction

  • Provide companionship through regular visits and conversations with residents

  • Assist residents with in-room activities, including crafts and letter writing

  • Transport residents to and from activities within the facility

  • Prepare and maintain activity areas before and after programs

  • Assist with outings, community walks, and special events

  • Maintain accurate and timely documentation in accordance with facility standards

  • Ensure compliance with HIPAA and all facility policies

  • Participate in quality assurance initiatives and regulatory surveys as needed

  • Coordinate with other departments to support resident engagement and programming

Requirements
  • Ability to understand and communicate effectively in English

  • Strong interpersonal and communication skills

  • Compassionate, patient, and empathetic approach to resident care

  • Ability to work independently and make sound decisions when needed

  • Creative mindset with a willingness to introduce new ideas and activities

  • Professional demeanor when interacting with residents, families, and staff

  • Basic computer skills; familiarity with Sigma Care and Microsoft Word is a plus

  • Interest or experience in arts, music, or recreational activities preferred
