Overview
A home health provider is seeking an Occupational Therapist to deliver skilled, in-home care to patients in Manhattan, NY. In this role, you'll work as part of a close-knit interdisciplinary team, collaborating with physical therapists, speech therapists, nurses, social workers, dietitians, and home health aides, while independently managing a flexible daily schedule.
What you'll do- Evaluate patients and provide skilled intervention focused on activities of daily living within the patient's home environment
- Support ADL progression, falls prevention, energy conservation, and personal care independence
- Implement treatment interventions such as adaptive equipment trials, compensatory strategies, ROM/strength progression, dynamic balance training, functional activity advancement, and caregiver education
- Participate in developing and periodically revising a comprehensive, interdisciplinary plan of care, coordinating closely with the patient's physician on condition changes
- Instruct patients and families in activities of daily living
- Establish household management routines
- Instruct patients and families in the use of prosthetic, orthotic, and assistive devices
- Educate patients and caregivers on appropriate adaptive equipment use
- Perform other duties as assigned
- Graduate of an accredited occupational therapy program
- Licensed by and currently registered with the New York State Education Department
- One year of practical experience as a licensed occupational therapist
- Strong computer skills, including the ability to use the EMR system in place
- Strong interpersonal, organizational, time management, and written and verbal communication skills
- Ability to work independently and complete paperwork accurately and in a timely fashion
- Compliance with privacy and HIPAA laws, with respect for patients' rights, dignity, and belongings
- Valid driver's license and an automobile with current insurance coverage in good working condition, available for work use
- Willingness to travel throughout the assigned county
- Willingness to submit to a physical examination, criminal background check, and drug testing
