Qualifications

• Active New York State Physical Therapy license (or eligibility) and graduation from an accredited physical therapy program (DPT or equivalent) are required.
• Prior experience in home health, outpatient orthopedics, or rehabilitative care is preferred, along with familiarity with evidence-based practice guidelines.
• Valid driver’s license or reliable transportation within Queens, NY, and the ability to meet physical demands of home visits (lifting, transfers, ambulation support).

Role Description This full-time Homehealth Physical Therapist role is an on-site position based in Queens, NY, serving patients in their homes and local community settings. The therapist will perform comprehensive patient evaluations, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, function, and quality of life. Day-to-day responsibilities include coordinating care with referring providers, documenting clinical findings and progress in accordance with regulatory and company standards, and educating patients and families on exercises, safety, and injury prevention.

The role also involves collaborating with an interdisciplinary team, managing a caseload efficiently, and participating in ongoing clinical development to maintain high standards of care.
