Elderwood at North Creek is seeking an experienced and engaging Director of Rehabilitation Services to lead our therapy department and help ensure exceptional outcomes for the residents and families we serve.

This is an opportunity to step into a hands‑on leadership role where your clinical expertise, organizational skills, and ability to inspire others will directly impact the quality of rehabilitation services provided across our community.

As Director of Rehabilitation Services, you’ll oversee the daily operations of the therapy department, support and develop your team, collaborate with interdisciplinary partners, and help ensure therapy services are delivered with compassion, clinical excellence, and compliance with federal and state requirements.

Elderwood at North Creek is an 82‑bed skilled nursing facility offering comprehensive, personalized orthopedic, cardiac, and post‑surgical rehabilitation programs, including physical, occupational, and speech therapy. Our services also include long‑term care, memory care, palliative and respite care, subacute rehabilitation, outpatient therapy, and Adult Medical Day Services.

Responsibilities
  • Lead and supervise the rehabilitation department, including therapists, therapy assistants, and rehabilitation aides.
  • Oversee the provision, documentation, and billing of therapy services in accordance with federal and state regulations, facility policies, and applicable practice acts.
  • Maintain a clinical caseload while providing department leadership and oversight.
  • Evaluate residents’ rehabilitation potential and develop appropriate treatment plans in collaboration with physicians and the interdisciplinary care team.
  • Ensure timely completion and accuracy of evaluations, progress notes, care plans, discharge summaries, MDS‑related documentation, and other required clinical records.
  • Communicate with residents and their representatives regarding rehabilitation goals, treatment plans, expected outcomes, and participation in therapy.
