Responsibilities- Assist members in person or by phone with challenges such as depression, anxiety, substance use, mental health concerns, family and child-related issues, domestic violence, and job jeopardy. Provide assessments, referrals, and follow-up support.
- Offer ongoing support and interventions for members admitted for psychiatric care, detoxification, and rehabilitation.
- Collaborate with families, educators, therapists, and community partners to ensure cohesive care for clients.
- Provide members with personalized referrals to community-based resources and behavioral healthcare providers.
- Utilize QNXT, Salesforce, DMS, ICIS, V3, and other databases, as needed, to complete tasks, authorizations, and documentation.
- Document services and progress in accordance with clinical and regulatory standards.
- Review and Process Behavioral Health Prior Authorizations, including for Intensive Outpatient Programs (IOP) Partial Hospitalization Programs (PHP), Transcranial Magnetic Stimulation (TMS), and Applied Behavior Analysis (ABA) prior-authorizations.
- Provide emotional well-being workshops, grief support, and crisis support, both in the field and virtually.
- Participate in health fairs and tabling events. Provide information on healthy lifestyles and encourage members to participate in healthy living programs offered through Wellness/MAP or vendor programs such as Weight Watchers, Noom, Flytehealth, and Virta, etc...
- Participate in member-support programs, such as Post-ER Outreach, Maternal Health Program, Adolescent Crisis Intervention Program, and other initiatives as needed.
- Collaborate with MAP/Wellness nurses, health educators, and nutritionists.
- Perform additional duties as assigned by management.
