Key Responsibilities- Receives referrals and reviews application materials for potential intakes to Children’s Crisis Residence, completing virtual and in‑person assessments and collecting information to determine individualized needs and make recommendations regarding necessary supports for youth.
- Facilitates admissions in collaboration with a multidisciplinary team, families, external providers and case managers; develops written, person‑centered, trauma‑informed Individual Support Plans (ISPs) and crisis plans that address the youth’s needs in accordance with agency philosophy and OMH guidelines.
- Regularly collaborates with program leadership and staff to promote the excellence of the program and protect the health and safety of youth, ensuring services are delivered in compliance with Federal, State, Local and Office of Mental Health requirements.
- Conducts clinical assessments of youth supported throughout their stay, evaluates progress, and designs effective data collection and charting systems to monitor outcomes.
- Ensures appropriate implementation of person‑centered treatment, therapeutic interventions and ISPs; monitors and reviews implementation, data collection and charting, and modifies plans to increase effectiveness.
- Actively participates in treatment implementation, including facilitation of therapeutic activities and observation of staff to provide feedback and ensure consistency in documentation.
- Provides individual, group and family therapy on an ongoing basis during the youth’s stay and may share clinical information with new providers following discharge.
- Completes and maintains all clinical documentation and paperwork in appropriate systems on a timely basis, in accordance with laws, regulations and established processes.
- Provides therapeutic support during youth crisis, using de‑escalation and intervention supports and supporting staff in implementing crisis plans.
- Develops professional, empathetic relationships with families and service providers, acting as a representative of the program, advocating for youth and fostering communication between supports.
- Provides education to program staff to ensure understanding and appropriate implementation of ISPs and crisis plans, and consistent data collection.
- Regularly participates in case conferences, discharge meetings and other interagency meetings regarding youth, providing insights and receiving updates.
- Serves as Administrator‑on‑Call (Available/On‑Call Supervisor) with program leadership, providing on‑call support to staff and addressing significant issues as needed.
- Coordinates with the director and program team regarding discharges and transitions, sharing information about supports and interventions with families and external providers.
- Collaborates with program leadership to document and report errors, incidents and corrective actions in compliance with applicable requirements.
- Stays abreast of best practices in mental health and counseling, identifies new methods to incorporate into plans, and develops staff training and education.
- Complies with all regulatory agency requirements, including HIPAA.
- Performs all other duties, as assigned.
- Current license and registration issued by the New York State Education Department (NYSED) as Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC) or Licensed Marriage & Family Therapist (LMFT) (or a similar discipline authorized to provide direct mental health services).
