Job Duties and Responsibilities
include the following. Other duties may be assigned:
- Ensure that each individual has a current and accurate Life plan/ ISP and LOC on file. Maintain and review history book documentation in accordance with Block Institute’s Policies, Protocols and Practices on at least a quarterly basis. Timely submission of the ISS quarterly history book review documentation to Individual Support Specialist Supervisor
- Provision of individual program planning at assigned residence(s). This includes but is not limited to the assessment, development, and implementation and monitoring of functional, clearly written and creative training programs. Within Intermediate Care Facilities, prepare and maintain Comprehensive Functional Assessments (CFA) and Prioritized Needs’ Lists.
- Create and update Residential Habilitation Plan/ Staff Action Plan on a semi-annual basis or as necessary. Ensure that Residential Habilitation plan and habilitation services identified correspond with at least one CQL Valued Outcome as identified on the individual’s ISP/ Life Plan document.
- Create a schedule for and conduct I AM Assessments, semi-annual Life Planning Team meetings in conjunction with residential clinical team, care managers, day program or employment representatives, and advocates and/or involved family members. Complete written minutes to accompany Residential Habilitation plan/ Staff Action Plan and reflect progress towards completing habilitation services and any changes made to the plan.
- The Individual Supports Specialist is responsible for training DSP staff on the implementation of services. If unavailable to complete training on services the Individual Supports Specialist should provide adequate resources for the Residential Management team to conduct training

- Minimum Bachelors Degree. Masters degree preferred
