Up to $80,000 per year
The Riverside Premier Rehabilitation and Healing Center is seeking a dedicated and compassionate Social Worker to join our interdisciplinary team. This is an excellent opportunity to work in a large, fast-paced rehabilitation and long-term care environment focused on delivering high-quality resident-centered care.

With a large bed capacity and a state-of-the-art rehab program, we provide services seven days a week and foster a collaborative environment that supports both professional growth and exceptional patient outcomes.
Social Worker Position OverviewThe Social Worker plays a key role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring safe and effective discharge transitions.
Social Worker Key Responsibilities- Assess residents’ social, emotional, and psychosocial needs
- Develop and implement individualized care plans
- Participate in interdisciplinary care plan meetings
- Coordinate admissions, transfers, and discharge planning
- Assist with securing home care services, durable medical equipment (DME), and community resources
- Provide support and guidance to residents and families throughout their stay
- Collaborate with clinical and administrative teams to ensure continuity of care
- Assist with financial documentation and resident education as needed
- Advocate for resident rights and quality of life
- Bachelor’s Degree in Social Work (BSW), Master’s Degree (MSW), or related field preferred
- Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred
- Strong communication, organizational, and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
- Knowledge of discharge planning, community resources, and care coordination
- Familiarity with PCC and MDS 3.0 is a plus
- Salary up to $80,000 annually (based experience)
