- Serve as a licensed behavioral health clinician advancing behavioral health integration in primary care using the Collaborative Care Model
- Provide evidence-based behavioral health interventions and psychoeducation
- Assess and systematically collect and track patient behavioral health data in a patient registry
- Work with the care team to develop patient plans of care
- Participate in in-person or virtual huddles discussing shared patients
- Use data and registries to meet care coordination needs across settings with patients, families, and other care providers
- Develop and maintain knowledge of community resources and managed care review processes
- Manage behavioral health care for individual patients with responsibility and accountability
- Document histories, assessments, and interventions accurately and timely in the electronic medical record
- Identify patient and family learning needs and provide adapted education using the teach‑back method
- Attend team meetings and case conferences to present and discuss patient disposition and outcomes data
- Coordinate care across inpatient, outpatient, home, skilled nursing facility, and school settings
- Assist with urgent and emergent patient care situations
- Assist with management of patients with chronic conditions, acute care episodes, and behavioral health challenges while promoting prevention and wellness
- Provide safe, ethical, patient- and family-centered, culturally congruent, evidence-based care
- Perform other related duties within scope, education, and training
Requirements
- Master's Degree
- Licensure in social work (LCSW or LMSW) or mental health counseling (LMHC)
- Experience working with individuals and families affected by mental illness
- Experience providing evidence-based psychotherapeutic approaches
- Ability to thrive within a complex medical system
- Working understanding of the healthcare system
- Ability to communicate effectively
- Excellent organizational and time management skills
- Ability to work effectively within a multidisciplinary team
- Ability to serve as a mentor and resource to others
Core Competencies
Demonstrates expertise in providing evidence-based behavioral health interventions and psychoeducation while effectively coordinating care across various settings. Holds licensure in social work or mental health counseling and possesses strong organizational, communication, and teamwork skills.
