Social Worker - Care Coordination

Position

Social Worker – Evening Shift (United States of America)

• Provide clinical and case management care to patients on a mobile crisis team setting.
• Conduct individual, group, and family therapy.
• Document services in a timely and high quality manner.

Evening Shift – 7.5 hours per day, 2 pm - 10 pm, including evenings, weekends, and holidays.

• Master’s degree in Social Work required.
• New York State registration/Certification as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
• Experience treating patients with concurrent substance abuse and mental illness required.
• Prior experience in a hospital or community health agency preferred.
• Experience with assigned patient population and utilization of PM Schedule required.

Salary range: $41.694/ hr - $50.88/ hr (commensurate with experience).
