Registered Dietitian; Per diem

North Westchester Restorative Therapy and Nursing has an opportunity for a Per diem Registered Dietitian.

Duties include:

• Assessments
• Care planning,
• Weight monitoring.

Good organizational skills and communication skills needed.

Responsibilities Include:

• Evaluate the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record, the hospital discharge papers, the interdisciplinary health care team, and family
• Apply knowledge of nutrition screening assessment to specific patient populations to define their nutritional needs
• Document pertinent findings in the medical record as per policy
• Provide quality nutrition counseling to patients, family members as indicated
• Implement and evaluate nutrition care plans for effectiveness; recommend changes as needed
• Use professional judgment to adapt as well as apply guidelines, protocols plus professional standards of care to the new changing needs of the patient
• Evaluate the need for enteral and parenteral nutrition support; providing recommendations as indicated
• Participate in interdisciplinary care plan meetings; communicating effectively with members of the health team
• Complete Minimum Data Set (MDS) documentation as requested by the facility
• Assist with discharge planning
• Menu planning as needed
• Knowledge of SIGMA Care preferred

Salary: $38.00 - $40.00 per hour
