Community Health Worker, Bureau of Queens Neighborhood Health

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and healthcare organizations.

CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

• Maintain communication with local organizations and stakeholders to promote NYC Health Department programs and resources and to collect feedback on community needs.
• Engage with community partners to promote health services, resources, and education.
• Develop and maintain relationships with a broad variety of community partners.
• Represent the bureau and the Jamaica Neighborhood Health Action Center at health fairs, meetings, and other external events occurring throughout the community.
• Deliver presentations and workshops to diverse audiences throughout the community.
• Maintain and report data that demonstrate community activities and results.
• Support Bureau-wide programs and initiatives.
• Participate in Just in Time training with NYC Health Department divisions that serve as a lead for a local/hyperlocal emergency response.
• Distribute information and coordinate information sessions to community residents and businesses in response to emergency incidents.
• Support agency efforts in building resilience in identified communities to mitigate impact from localized emergencies such as legionella outbreaks; conduct messaging, communicating and other efforts to make community aware of climate health preventive measures.

PREFERRED

SKILLS:

• 2+ years of community work experience.
• Excellent organizational skills and highly detailed oriented
• Excellent oral and written communication skills
• Demonstrated ability to effectively establish and maintain working relationships with culturally diverse community groups.
• Knowledge of Queens local communities and local organization leadership, specifically Jamaica and surrounding areas.
• Comfortable speaking in public.
• Bilingual (written and spoken) Spanish, Bengali, Mandarin, or other language proficiency encouraged to apply
