Provide support and services to assist consumers with maintaining sobriety and psychiatric stability. Provide linkages for obtaining substance use and/or psychiatric treatment within a community environment.
Duties and Responsibilities- Encourages and supports consumers with sobriety, stability, and treatment linkages.
- Coordinates services with Clinical Coordinator, Supervisor, and Care Managers.
- Visits consumers at least twice monthly or as needed.
- Teaches independent living skills to sustain housing and prevent homelessness.
- Develops Individualized Recovery Plans and monitors progress on goals.
- Ensures fire safety equipment is present and educates residents on use.
- Teaches emergency protocols (medical, psychiatric, fire, food, etc.).
- Acts as liaison with landlords, teams, and mandated treatment providers.
- Completes timely documentation and attends required meetings.
- Bachelors degree in Human Services (BSW preferred).
- 3+ years experience with target population.
- Organizational, Communication, Critical Thinking, And Judgment Skills Required.
- Valid drivers license and car with insurance.
- Evening/weekend availability required.
- Completion of mandatory trainings and fingerprinting.
- Adheres to HIPAA and agency compliance plan.
