Regional Rehab Manager

Job Purpose

Under the direction of the AVP of Care Delivery Operations and the Rehabilitation Director, the Regional Rehab Manager is responsible for the supervision of the Physical Therapy and Occupational Therapy staff. The Regional Rehab Manager is responsible to ensure a high level of therapy is provided to all PACE members in order to have positive outcomes including low hospitalization rates, low fall rates, and that all members function at their highest level of ADL independence.

Responsible for supervision of Physical and Occupational Therapists including hiring, firing, disciplinary in conjunction with thorough record reviews, audits, field visits and consultation with supervisors in other disciplines. In conjunction with the AVP of Care Delivery and the Rehabilitation Director, develops policies and standards for rehab services. Determines in‑service needs of Center Light PACE staff regarding rehab services. Assures that Physical and Occupational Therapy staff documentation and communications, both written and verbal, are timely and of good quality.

Participates in PACE Supervisory activities including, but not limited to, interdisciplinary case conferences, quality assurance, clinical record audits, utilization review, development of new services, and developing Performance Improvement Plans. Participates in planning and implementation of rehab relevant educational programs for all staff. Plans and executes studies and community surveys as appropriate. Coordinates Rehabilitative Services with other disciplines to achieve the most comprehensive care possible.

Collaborates with Administration and AVP of Care Delivery and the Rehabilitation Director, on budget and equipment issues, staffing pattern and evaluations, patient care concerns, and coordination of Home Therapy cases. Evaluates and treats Rehabilitation patients in specific area of expertise as appropriate. Under the direction of the CSD, AVP of Care Delivery, RED, or based on the needs of any site, the Regional Rehab Manager will visit community resources (SNF, MD offices, Out‑PT Rehab clinic) to assess PACE members, Discuss POC’s, develop business partnerships or to promote PACE services.

Performs all special assignments as assigned. Only acts within the scope of the individual’s authority to practice. Meets a standardized set of competencies for the specific position description established by the PACE organization before working independently. Acting member of the IDT.
