REPORTS TO:

Director of Rehabilitation / Occupational Therapist

San Simeon by the Sound Center for Nursing and Rehabilitation is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team.

The COTA works under the direction and supervision of a licensed Occupational Therapist to provide skilled occupational therapy services to residents. The ideal candidate is dependable, resident-focused, and committed to helping individuals improve their independence and quality of life.

• Provide occupational therapy treatments according to established plans of care developed by the Occupational Therapist.
• Assist residents in improving their ability to perform activities of daily living, including dressing, grooming, bathing, feeding, and mobility-related tasks.
• Implement therapeutic exercises, activities, and interventions designed to improve strength, coordination, balance, endurance, and functional independence.
• Monitor resident progress and communicate changes in condition or performance to the supervising Occupational Therapist.
• Accurately and timely document treatments, resident responses, and progress in accordance with facility, regulatory, and reimbursement requirements.
• Educate residents, families, caregivers, and staff regarding therapeutic techniques and safety recommendations as appropriate.
• Participate in interdisciplinary care planning and collaborate with nursing, rehabilitation, social services, and other departments.
• Maintain resident safety and follow infection-control, HIPAA, and facility policies and procedures.
• Assist with adaptive equipment and techniques designed to improve resident independence and safety.
• Perform other related duties within the COTA scope of practice as assigned.

• Current New York State authorization/licensure to practice as a Certified Occupational Therapy Assistant, as applicable.
• Graduate of an accredited Occupational Therapy Assistant program.
• Current CPR certification as required by the facility.
• Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary healthcare team.

$30.00 per hour.
