An exempt position responsible for providing professional therapeutic services to patients and families including psychotherapy, crisis intervention, and coordination of community services/resources under supervision. Performance of duties may be autonomous and self-directed as part of an interdisciplinary team.

• Conducts and documents comprehensive, culturally sensitive psychosocial assessments, develops treatment plans, and provides psychotherapy and crisis intervention services to individuals and families under supervision
• Coordinates services with appropriate clinic and community resources under supervision
• Provides information and education about health/mental health issues to individual patients and families under supervision

• Master’s degree in social work from an accredited school
• Three years of experience in post-graduate mental health setting and broad background with diverse patient populations, preferably in medical setting preferred
• Currently licensed to practice social work in New York State

• Knowledge of professional therapeutic principles, methodology, and ethics and of human psychosocial development within the family, community, and culture
• Knowledge of the use of therapeutic relationship to foster patient involvement
• Familiarity with brief therapy theories/techniques and therapeutic process
• Understanding of techniques for facilitating client motivation to change behavior
• Knowledge of health and social issues impacting diverse clients and their well-being

• Skill in crisis intervention
• Skill in client advocacy
• Skill in case management

• Ability to interact effectively as member of interdisciplinary health care team
• Ability to identify and utilize community resources
• Ability to communicate appropriately with diverse patient population
• Ability to consistently complete required documentation accurately and precisely

Standard office equipment including computer hardware and software to access community resource database, electronic medical record, Microsoft Word/Excel/Power Point
