Behavioral Health Screener; LMSW​/LCSW

Community Healthcare Network is seeking for a Full-Time Behavioral Health Screener who will work as an integral part of the Behavioral Health treatment team providing intake services under the direction of the AVP of Behavioral Health, BH Regional Clinical Supervisors and treating psychiatric providers.

Responsibilities include, but are not limited to:

• Provides diagnostic assessments and screening services to individuals interested in becoming participants in behavioral health services.
• Utilizes appropriate scoring tools to identify baseline for provision of care.
• Provides face-to-face BH screenings or therapy visits to at least 35 patients per week.
• Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy.
• Proactively refers patients to a different level of care.
• Coordinates on daily basis with BH Regional Supervisors to discuss clients screened and where to assign..
• Utilizes MI to assess readiness for BH services.
• Billable documentation is locked within 24 hours.
• Maintains small caseload of 10 patients, discusses cases in clinical supervision.
• Appropriately schedules patients for psychiatric, behavioral health, or other services.
• Uses evidence based models of treatment with patients on caseload (eg Cognitive Behavioral Therapy, Eye Movement Desensitization and Reprocessing, Interpersonal Therapy, Seeking Safety, Dialectical Behavioral Therapy, Problem Solving Therapy, Behavioral Activation, etc.).
• Performs other related duties, as assigned.

• M.S.W Degree.
• Licensed Master Social Worker or Licensed Clinical Social Worker.
• Bilingual Spanish strongly preferred.
