Education:

• Master’s Degree in Occupational Therapy from an accredited institution.

Experience:

• Experience working with individuals with Serious Mental Illness (SMI) and/or substance use disorders.
• Experience working in supportive housing, community-based programs, or scattered-site housing models preferred.
• Experience conducting health assessments, developing individualized care plans, and supporting community integration.
• Strong understanding of trauma-informed care and person-centered practices.
• Knowledge of chronic disease management, preventive healthcare, and behavioral health services

Certifications/Licenses:

Current New York State Occupational Therapist License required

The Occupational Therapist will join ICL’s Health Connect Program within Supported Housing, providing preventive healthcare support and chronic disease management services to individuals living in supportive housing settings. This role focuses on helping clients maintain independence, improve overall wellness, and successfully manage their mental health, physical health, and recovery goals through a trauma-informed and person-centered approach.

The Occupational Therapist works closely with a multidisciplinary team to conduct assessments, develop individualized care plans, strengthen daily living skills, promote community integration, and connect clients to healthcare and community resources that support long-term stability and recovery.

• Conduct comprehensive health assessments addressing clients’ psychiatric, medical, and social needs.
• Monitor chronic health conditions and support symptom management and preventive healthcare interventions.
• Develop and implement individualized care plans that promote independence and community integration.
• Collaborate with multidisciplinary professionals, including social workers, nurses, clinicians, and peer specialists, to provide holistic care.
• Provide health education and wellness training to clients and staff, focusing on prevention and chronic disease management.
• Participate in the Clinical High-Risk Framework and High-Risk Meetings to assess needs, identify priority levels, and support timely interventions.
• Encourage client engagement in treatment, wellness initiatives, and health-promoting activities.
• Coordinate with healthcare providers and community resources to improve client outcomes and continuity of care.
• Teach and reinforce independent living skills, including home management, budgeting, meal preparation, and grocery shopping.
• Assist clients with accessing transportation, healthcare services, social services, and other community resources.
• Promote social engagement and community participation to strengthen support systems and overall well-being.
• Maintain accurate and timely documentation in the Electronic Health Record (EHR) and other required systems.
• Provide advocacy, support, and coaching to help clients achieve greater independence and quality of life.
• Perform additional duties as assigned.
