Location: New York
We are seeking a Per Diem Speech Language Pathologist for a Skilled Nursing Facility. The SLP PRN will work closely with the adult population with complex communication needs. Great opportunity to join a collaborative rehab team in a supportive SNF environment.
Job Responsibilities- Conduct comprehensive assessments and provide evidence-based treatment of speech, language, cognitive and swallowing abilities.
- Develop individualized treatment plans tailored to each patient’s needs.
- Collaborate with families, educators, and healthcare professionals to support patient progress.
- Complete accurate and timely documentation including daily notes, progress reports, evaluations, and discharge summaries.
- Stay current with advancements in speech therapy practices and medical terminology.
- Ensure compliance with facility protocols and state/federal regulations.
Qualifications:
- Master’s degree in Speech Language Pathology
- Valid NY SLP License.
- Strong communication and organizational skills contributing to a collaborative environment
- Knowledge of dysphagia and cognitive rehabilitation preferred
- Experience in a skilled nursing facility preferred
- Experience with Net Health/Rehab Optima is a plus
