New York Medical & Vascular Care (NYMVCare) is seeking a motivated and patient-focused Physical Therapist to join our growing outpatient healthcare team.
NYMVCare is an established multispecialty medical practice serving communities throughout Brooklyn and Queens. Our Physical Therapy team works closely with physicians and other specialties to provide coordinated, individualized care focused on improving mobility, function, independence, and overall quality of life.
Responsibilities- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercise, functional training, mobility and gait training, balance activities, and other appropriate interventions
- Help patients improve strength, mobility, function, and independence
- Monitor patient progress and adjust treatment plans as clinically appropriate
- Educate patients and caregivers regarding exercises, mobility, safety, and home programs
- Maintain accurate and timely documentation in the electronic medical record
- Communicate patient progress and concerns with physicians and other members of the care team
- Collaborate with PT staff, medical providers, and support staff to provide coordinated patient care
- Maintain a safe, professional, and patient-centered treatment environment
- Graduate of an accredited Physical Therapy program
- Current New York State Physical Therapist license or eligibility for licensure
- Strong clinical, communication, and interpersonal skills
- Ability to develop positive relationships with patients and work effectively as part of a team
- Strong organizational and documentation skills
- Outpatient, orthopedic, neurological, rehabilitation, or medically complex patient experience is preferred
