Manorville, NY 11949Job Summary
Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our home health team in Manorville, NY. In this role, you will deliver individualized, evidence-based therapy to patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.
Job Responsibilities- Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
- Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
- Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
- Collaborate with the clinical team to ensure coordinated and effective treatment
- Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
- Active NY Physical Therapist / Physical Therapist Assistant License (PT/PTA)— required
- Basic computer proficiency for electronic documentation
- Strong clinical assessment and interpersonal communication skills
- Ability to work independently in a home health environment
- Valid NYdriver's license and reliable transportation for home visits
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
- Job Type: Full-time, Part-time, Per Diem

Manorville, NY 11949 - Salary: $65 - $80 per hour
- Schedule Flexibility:
Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
