This is a regional therapy role—not a traditional home health position. Most services are delivered in PACE sites and outpatient/clinic settings, with home or community visits completed on a limited, participant-needs basis. In addition to direct care, the role has a meaningful focus on quality reviews, documentation, mentoring, and continuous improvement.
- Evaluate participants and provide individualized physical therapy treatment to improve mobility, function, safety, comfort, and independence.
- Deliver care across PACE sites and outpatient/clinic settings, with limited home, nursing home, or community-based visits when participant needs require them.
- Develop and update therapy plans of care in collaboration with participants, caregivers, and the interdisciplinary care team (IDT).
- Document evaluations, progress notes, care plans, treatment activity, and other required information accurately and timely in the EMR.
- Serve as an active member of the IDT and contribute therapy expertise to care planning, service determinations, transitions, and participant outcomes.
- Support quality assurance through chart reviews, audits, documentation reviews, and recommendations that strengthen service delivery and compliance.
- Partner with the Regional Rehabilitation Manager and Director of Rehabilitation on quality improvement initiatives, regulatory readiness, and operational needs.
- Provide clinical guidance, mentoring, orientation, and direction to Physical Therapist Assistants, students, and new team members as assigned.
- Promote safe clinical practices, appropriate use of equipment and supplies, and a clean, participant-centered treatment environment.
- Travel within the assigned region to support PACE locations and participant care needs.
Required Qualifications:
- Master’s or Doctoral degree in Physical Therapy from an accredited program.
- Current and valid New York State Physical Therapy license/registration.
- At least one year of experience working with a frail or elderly population; candidates with less than one year may qualify with appropriate PACE-provided training in accordance with program requirements.
- Ability to practice within all applicable professional, state, and regulatory requirements.
- Ability to travel within the assigned region, including between PACE sites and, when clinically necessary, participant homes, nursing homes, or other care settings.
- Ability to establish effective relationships with participants, caregivers, colleagues, and interdisciplinary partners.
- Ability to meet the physical requirements of the role, including assisting with participant mobility and safely lifting/pushing/pulling up to 50 pounds as required.
- Medical clearance for communicable diseases and required immunizations prior to direct participant contact.
- Two or more years of physical therapy experience.
- Experience across multiple clinical settings such as outpatient, rehabilitation, home health, or acute care.
- Geriatric rehabilitation experience.
- Knowledge of PACE regulations and interdisciplinary care models.
- Experience with Care Compass or other electronic medical record systems.
- Bilingual skills based on the participant demographics of the assigned region.
