Location: New York

A health center dedicated to community support is seeking a HPAC Patient Navigator to conduct outreach and connect low-income individuals with HIV prevention services and primary care. The successful candidate will work closely with diverse populations, assist clients in accessing healthcare, and conduct health education activities. A high school diploma, relevant experience, and fluency in Spanish or Mandarin are highly preferred.
