Care Source is seeking an Intake Nurse (RN) for Brooklyn, NY to assess eligibility for the program and explain services to potential members and families. The RN will collaborate with the intake/outreach and clinical teams to ensure appropriate service delivery and a seamless transition to managed long term care.
The role requires bilingual abilities in Spanish, Russian, or Chinese, an Associate's degree in Nursing, and at least one year of acute care experience; home care/MLTC intake experience
