The Care Manager Assistant provides face-to-face visits, ongoing support and care coordination to members. Responsibilities include face-to-face visits, communication and follow up with members, and reviewing the person-centered service plan with member/members' family and coordination of covered and non-covered benefits. Fieldwork required.

• Conduct multiple types of mandatory face-to-face visits based on need
• Schedule members due for face-to-face visits
• Meet weekly productivity expectations
• Complete necessary documentation, scripts, activities and follow-ups – ensure all items are addressed at the time of visit
• Understand and support company-wide goals based on direction
• Participate in weekly team meetings and ongoing training
• Other duties as identified as a necessary part of employee’s role

• Able to travel and conduct home visits
• Knowledge of member assessment procedure
• Excellent interpersonal skills – ability to speak effectively with members and employees of the agency
• Demonstrates the ability to effectively communicate information with members of all ages and socioeconomic levels
• Ability to read and interpret documents, understand and execute workflows

• The appropriate background and experience in Care Management and/or MLTC services
• Bilingual preferred (Cantonese/Mandarin)
• Bilingual preferred (Spanish)

Pay $29-$31 per hour
