About the Role:

As a Bilingual Community Outreach Representative at Personal Touch Home Care of N.Y. Inc, you will play a crucial role in connecting with our diverse community, promoting our services, and building relationships with patients and families. With a strong focus on personalized care, we are seeking a dedicated and compassionate representative who can effectively communicate our services to our bilingual audience.

Responsibilities:
  • Conduct community outreach and education programs to promote our services to patients, families, and healthcare providers
  • Build and maintain relationships with community organizations, health centers, and social services agencies to identify potential partnerships and referrals
  • Identify and address the social, emotional, and physical needs of patients and families, and provide personalized support and resources
  • Develop and maintain a strong understanding of our services, including home health and home care programs, to effectively communicate our offerings to the community
  • Collaborate with our multidisciplinary team to develop individualized care plans that meet the unique needs of each patient
  • Conduct language-specific outreach and education programs to ensure that our bilingual patients and families receive the care and support they need
Requirements:
  • Strong communication and interpersonal skills, with the ability to effectively communicate with patients, families, and community organizations
  • Bilingual in English/Spanish and strong reading, writing, and speaking skills
  • Ability to work independently and as part of a team to achieve goals and objectives
  • Strong problem-solving and organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously
