Family Health Centers at NYU Langone is seeking a Counselor to join our Brooklyn team, Monday–Friday 9am–5pm. Bilingual Spanish or Mandarin speaking is required to support diverse communities.
You will screen clients for social determinants of health, perform eligibility assessments for the 1115 Medicaid waiver, assist with benefit applications, provide short-term counseling, and connect clients to needed services, documenting in EPIC and coordinating with partner organizations.
