Patient Care Coordinators who are
experienced as Patient Care Coordinators
and who
speaks FLUENT English and Spanish and Creole
for our Brooklyn office.
Starting salary $24 - $27 per hour (depending on experience)

Qualifications:
Experienced patient care coordinator with a minimum of at least one year of experience in home care coordination.

Excellent telephone and communication skills
Excellent organizational skills
Ability to maintain accurate schedules, notes and records
Good computer skills
Good interpersonal skills
