Nurse Supervisor, Care Management
We are seeking an experienced and dedicated Nurse Supervisor to lead our Care Management team. This role offers the perfect blend of leadership and clinical expertise, allowing you to make a significant impact while maintaining a work-life balance. Your proficiency in discharge planning and utilization review will be highly valued as you guide our department to achieve exceptional patient care outcomes.
Responsibilities- Provide Associate Director level supervision to the Care Management Department
- Lead and guide discharge planning and utilization review processes
- Ensure high quality patient care outcomes through effective supervision
- Minimum 2 years of experience in discharge planning and utilization review
- CCM & PRI certified
- Bachelor’s degree required;
Master’s degree preferred
- Permanent Salary Range: $115,000 - $140,000 annually

- Schedule:
9:00 AM – 5:30 PM (Primarily Monday to Friday; some weekends required)
