Position: DIRECTOR-PCM DEPARTMENT
Location: New York

The Director - PCM provides leadership and oversight of incident management, investigations, regulatory reporting, corrective action monitoring, risk reduction, trend analysis, and other PCM functions across agency programs. This position provides leadership and supervision to assigned staff while ensuring regulatory compliance, operational accountability, investigative quality, continuous quality improvement, and the consistent application of PCM standards across agency programs.

While this position’s primary assignment is to lead incident management and investigative activities across all Saint Dominic’s Family Services programs and services, the Director - PCM may also oversee or conduct audits, compliance reviews, program evaluations, and perform other PCM responsibilities based on agency and departmental priorities.

Duties & Responsibilities:

Leadership, Supervision& Performance Management

Supervise, mentor, develop, and evaluate assigned staff

Assign, prioritize, and monitor incident management activities, investigations, regulatory reporting, corrective action follow-up, and other PCM activities

Conduct supervision, coaching, performance evaluations, and professional development for assigned staff

Review and approve investigative reports, incident-related reports, corrective action plans, regulatory submissions, and other work products prepared by assigned staff, as applicable

Monitor staff productivity, quality, timeliness, and performance metrics

Promote accountability, collaboration, and continuous quality improvement throughout assigned PCM functions

Incident Management,Investigations & Regulatory Oversight

Oversee incident management, investigations,regulatory reporting, and related PCM activities across all agency programs

Assign, oversee, and conduct investigations involvingincidents, allegations, complaints, and regulatory concerns

Review investigative reports to ensure findings areobjective, thorough, accurate, supported by evidence, and completed timely

Ensure incidents, investigations, regulatorynotifications, corrective actions, and required follow-up are completed timelyand in accordance with all applicable Federal, State, and local regulatory requirements, including but not limited to the NYS Justice Center, OPWDD, OMH,ACS, DOH, OCFS, NYSED, and other relevant oversight entities

Develop and improve systems, processes, tools, and workflows that support efficiency, consistency, accountability, and regulatory compliance

Coordinate and facilitate Incident Review Committee,At-Risk Committee, and other incident-related meetings, as applicable

Analyze incident trends, investigative findings, and corrective actions to identify risks, prevent recurrence, and recommendimprovements

Reporting, Collaboration& Organizational Support

Overseedashboards, trend analysis, KPIs, and executive reporting related to incidents,investigations, corrective actions, and identified risks

Provide technical assistance, guidance, and training to program leadership and staff regarding incident management, investigations, corrective actions, and regulatory expectations

Collaborate with Collaborate with Operations, Human Resources, Legal, Information Technology, and other departments to strengthen incident response, regulatory compliance, risk management, and continuous quality improvement

Participate in leadership meetings, committees, regulatory reviews, and otherorganizational initiatives as assigned

Flexibility &Operational Support

Adjust workload priorities and departmental activities based on operational needs, regulatory changes, and emerging issues

Provide cross-functional leadership,coverage, and support based on operational needs and departmental priorities

Perform other duties and responsibilities as assigned

Education & Experience
  • Bachelor’s degree in Social Work, Psychology, Public Administration, Healthcare, or a related field required; equivalent combination of education and relevant professional experience may be considered in lieu of the degree.

Minimumof five (5) years of progressively responsible experience in compliance,investigations, auditing, quality improvement, healthcare, child welfare,behavioral health, education, or related regulatory environments

Minimumof three (3) years of supervisory or leadership experience preferred

Experience working within regulated environments (NYS Justice Center, OPWDD, OMH, ACS,DOH, OCFS, NYSED, and other relevant Federal, State, and local oversightentities) strongly preferred

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer

Abilityto lift up to 25 pounds if needed
