Case Management Supportive Services (CMSS)provides community-based HIV/AIDS case management, health education/risk reduction and the provision of food voucherservicesto individuals
People Living with HIV/AIDS (PLWHA), thathave income below 500% of Federal Poverty Level based on household size and are New York State residents. This dynamic model of case management, health education/risk reduction and food voucher services will actively address the multiple needs of PLWHA and remove the barriers that prevent retention in care and treatment.
This case management model serves as a bridge to help consumers learn to navigate the health care and support system and gain the knowledge and skills necessary to effectively self-manage their care. It also helps support consumer entry into the full continuum of care that is available. Case management efforts must focus on improving health outcomes,leadingto viral suppression.
SPH-1:
The SPH 1 Program provides twenty units of permanent Scattered-Site apartments to single adults living with HIV/AIDS who face significant barriers to independent living. The program serves individuals ineligible for benefits from the HIV/AIDS Services Administration (HASA), often due to exceeding the income limits or not meeting the eligibility criteria for financial assistance.
Units will be scattered primarily throughout Brooklyn in apartments that meet HUD criteria and are located near public transportation.

Manager do:- Understand all aspects of contract requirements and communicate this understanding to staff.
- Train staff in program responsibilities, processes, and procedures, and ensure that all staff meets contract requirements in their daily work.
- Participate in the grant writing process and other funding initiatives.
- Develop and maintain key community partnerships.
- Oversee the development of social marketing campaigns.
- Work with staff to set realistic weekly and monthly performance targets in accordance with contract requirements.
- Coordinate and supervise all day-to-day activities of direct reporting staff.
- Periodically observe direct reporting staff engaging with clients and/or supervisory staff.
- Conduct weekly client progress reviews with staff.
- Conduct periodic internal file review and implement Quality Assurance measures as needed to ensure quality service delivery to clients.
- Provide crisis counseling to clients as needed.
- Prepare contract reports and statistical information for both CAMBA management and funder use.
- Complete program forms and reports for the agency’s finance, payroll and human resources departments as required.
- Assist in the analysis of program and demographic data to make programmatic improvements.
- Organize onsite and offsite training opportunities for professional development of staff.
