Bilingual Spanish Team Lead, Field Care Coordinator; Hybrid

Responsibilities

• Lead and supervise the Field Care Coordinators (FCC) assigned to a specific geography.
• Ensure FCCs conduct home care management visits in accordance with departmental policies.
• Supervise, evaluate, and coordinate daily team operations to meet productivity goals related to quality, timeliness, accuracy, and consistency.
• Allocate, monitor, and assign resources while delegating and monitoring workloads.
• Implement and support continuous process improvement and quality initiatives.
• Provide in‑field support to FCC staff, including supervision, coaching, and training.
• Exercise judgment within defined procedures and policies to determine appropriate actions and administer departmental policies.
• Develop and monitor team goals; provide ongoing feedback and coaching; conduct annual performance reviews.
• Lead by example, foster an atmosphere of open communication, teamwork, and empowerment to make informed decisions.
• Assist leadership in directing, training, and developing team members, embodying the Trailblazer mindset.
• Build and develop relevant business relationships, critically think about departmental problems, and recommend solutions.
• Perform additional duties as assigned.

• High School Diploma or GED.
• Bilingual Spanish skills – full fluency required.
• Demonstrated professionalism with experience in direct or indirect leadership of people or projects.
• Experience in a role requiring independent critical thinking, time management, problem solving, and goal achievement.
• Ability to travel to assigned geography (downstate New York area – the 5 boroughs, Long Island, Orange County, and Westchester).
• Effective verbal and written communication skills with strong grammar, spelling, and punctuation.
• Proficient computer skills, including proficiency with Microsoft Outlook, Word, and Excel.

• Associate’s or Bachelor’s degree.
• Prior training experience.
• Experience managing field teams.
• Leadership skills with the ability to develop, direct, and support a team.
• Adept at planning, organizing, and executing strategies.
• Ability to build and enhance team productivity to achieve team goals.

Salary Range:
Greater New York City Area (NY, NJ, CT residents) – $67,205 to $95,680;
Other approved locations – $59,800 to $87,360. Compensation is contingent upon experience, education, licenses, and certifications. Full range of benefits includes medical, dental, and vision coverage; incentive and recognition programs; life insurance; and 401(k) contributions (subject to eligibility).
