Project Hospitality in Staten Island is seeking an Outreach Specialist for the Health Homes program. The role conducts home visits and community outreach across Staten Island to identify clients eligible for services and re-engage those lost to follow up.
You will work with community providers, document findings, and participate in events on weekends. A High School diploma is required and bilingual Spanish/English a plus; a valid driver's license is required to operate the agency vehicle.
