POSITION RESPONSIBILITIES

Project Coordination and Operations

  • Coordinate project activities across priority Bronx neighborhoods to support campaign goals and deliverables.
  • Manage and monitor project timelines, schedules, work plans, and implementation activities.
  • Coordinate day-to-day project operations supporting the Bronx Preeclampsia Prevention Campaign.
  • Facilitate collaboration among hospital partners, community-based organizations, doulas, community health workers (CHWs), faith-based organizations, WIC sites, libraries, housing organizations, and other community stakeholders.
  • Plan, schedule, promote, coordinate logistics for, and support implementation of in-person, virtual, and hybrid meetings, trainings, community education sessions, and outreach events.
  • Lead coordination of a minimum of 20 project activities annually in collaboration with project leadership and community partners.
  • Ensure project activities align with grant requirements, project objectives, and established performance measures.

Partner Engagement and Community Outreach

  • Develop, maintain, and strengthen relationships with community partners and stakeholders supporting maternal health and birth equity initiatives.
  • Identify, recruit, and onboard new partners to support campaign implementation and sustainability.
  • Convene multi-sector partners to share project updates, identify opportunities for collaboration, and address community needs.
  • Support culturally responsive and bilingual outreach efforts throughout the Bronx.
  • Represent the project at health fairs, community meetings, collaborative initiatives, and outreach events to increase project visibility and engagement.
  • Serve as a point of contact for project partners and stakeholders regarding project participation, activities, and resources.

Education, Resource Coordination, and Community Support

  • Coordinate distribution of project materials, educational resources, prevention toolkits, outreach materials, and approved campaign communications.
  • Support implementation of evidence-informed educational messaging regarding preeclampsia prevention, warning signs, prenatal care, and postpartum care.
  • Assist in connecting residents and families to appropriate maternal health, behavioral health, food security, housing, transportation, and social service resources.
  • Coordinate with healthcare providers, OB/GYN practices, midwifery programs, hospitals, and community partners to ensure consistent project messaging and resource utilization.

Communications and Meeting Management

  • Develop and maintain project communication materials, partner packets, outreach documents, referral resources, flyers, and meeting notices.
  • Coordinate meeting logistics, agendas, attendance tracking, minutes, action items, and follow-up communications.
  • Support project leadership in developing culturally responsive, plain-language educational and outreach communications.
  • Maintain regular communication with partners regarding project activities, deadlines, and participation opportunities.

Data Management, Reporting, and Compliance

  • Maintain accurate and organized records of project activities, partner engagement efforts, events, outreach activities, referrals, and resource distribution.
  • Track project performance indicators, including partner participation, event attendance, outreach outcomes, materials distributed, surveys collected, and community reach.
  • Maintain and update databases of partners, referral resources, stakeholder contacts, and project activities.
  • Prepare monthly project summaries and progress reports for review by the Project Director.
  • Assist with grant reporting requirements by drafting sections related to project implementation, partner engagement, community outreach, and performance outcomes.
  • Monitor project deliverables and support continuous quality improvement activities.
QUALIFICATIONS

Required Qualifications

  • Bachelor's degree in Public Health, Health Administration, Community Health, Business Administration, Human Services, Social Work, Project Management, or a related field; or an equivalent combination of education and experience.
  • Minimum of 2 years of experience in project coordination, program coordination, community outreach, public health, healthcare administration, or nonprofit program management.
  • Experience coordinating multiple projects, events, stakeholders, and competing priorities.
  • Strong organizational, project management, and time-management skills.
  • Excellent written and verbal communication skills.
  • Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, PowerPoint, and Teams.
  • Ability to collect, track, analyze, and report project data.
  • Ability to establish and maintain effective working relationships with diverse stakeholders and community partners.
  • Ability to be flexible to work occasional evening and/or weekend
  • Must be able to organize and attend regular events and training.
