Breaking Ground is seeking a Consumer Advocate, Outreach to serve people experiencing street homelessness with chronic medical conditions in Brooklyn. The role focuses on engaging clients to access social and medical services and secure housing solutions.
The Consumer Advocate, Outreach builds relationships via motivational interviewing, conducts assessments, and travels in the field to connect clients with services and housing options. A 35-hour work week and onsite field work are required.
