Job Description

Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Responsibilities
  • Assess the patient’s physiologic health status and conduct patient interview, explain policies, review charts and document findings.
  • Assess the patient’s psychosocial health status, elicit perceptions, determine coping mechanisms, identify cultural requirements and document the psychological plan of care.
  • Identify patient outcomes, develop criteria for measurement, set individualized goals and document outcomes.
  • Formulate the plan of care and prioritize activities to achieve expected outcomes, coordinate cost‑effective use of supplies, equipment and medication.
  • Implement the plan of care, maintain vigilance over patient safety, administer treatments, use equipment correctly, provide emotional support, and document interventions.
  • Evaluate care provided for patient outcome, perform variance analysis and implement quality improvement initiatives.
  • Collaborate with the care team and physicians in planning and carrying out treatment regimens, delegate assignments appropriately and keep management updated.
  • Provide grade I‑IV decubitus care and preventive skin care measures.
  • Administer medications correctly and safely, including preparation of local solutions and IV drips, and educate patients on drug interactions.
  • Participate in patient and family education, recognize teaching opportunities, provide discharge planning and evaluate teaching effectiveness.
  • Perform related duties as required; meet physical demands such as standing, walking, stooping, lifting, and pushing up to 200 lbs.
  • Qualifications

    Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010, and obtain a BSN degree within five (5) years of employment date.

    Current license to practice as a Registered Professional Nurse in New York State. BCLS, ACLS preferred.
