Concerto Care Advanced Practice Provider (Nurse Practitioner & Physician Assistant)

We offer in‑home clinical services delivered through a multidisciplinary team. Our wraparound care model supplements primary care, preventing ER visits and hospitalizations.

• Deliver compassionate, individualized, holistic patient care including chronic condition management and urgent care.
• Collaborate with multidisciplinary team for transitions of care, social determinant interventions, behavioral health, and medication management.
• Provide in‑person care in private homes, congregate living facilities, or other home settings and virtually via telehealth.
• Maintain comprehensive medical records, documentation, coding, and medication reconciliation.
• Participate in care team huddles, operational huddles, and clinical partnership meetings.
• Coordinate care with external providers and build ongoing relationships with PCPs and other partners.
• Enjoy a healthy work‑life balance with limited after‑hours responsibilities; rotating on‑call coverage is strictly telephonic.
• Other market‑specific duties as assigned.

• Graduate of an accredited nursing or physician assistant school and active licensure in New York.
• ANCC or AANP certification for Nurse Practitioners; board‑certified Physician Assistant. PMHNP/NNP not eligible.
• Active DEA licensure and state‑specific controlled substance registration.
• Advanced training in geriatrics or palliative medicine preferred.
• Basic life support (BLS) certification required.
• Clinical experience caring for aging adults and adults with complex care needs.
• Experience caring for patients in the home setting (private homes and/or congregate living facilities).
• Experience providing virtual care via telehealth.
• Average to advanced computer and software skills.
• Ability to travel as needed to locations with limited public transportation; proof of vehicle liability and property damage insurance required.
• Bilingual speakers preferred.
• Driver requirements: licensed for minimum 5 years; no vehicle‑related suspensions or more than 2 moving violations in last 5 years.

Base Salary: $135,000–$160,000 plus annual bonus. Total rewards package includes full healthcare coverage, 401(k) with match, and other health, wellness, and financial benefits.
