New Vanderbilt Rehabilitation and Care Center is hiring a Registered Nurse (RN) Care Coordinator for the vent unit, offering up to a $5,000 sign-on bonus. This full-time position (11a-7p) involves providing direct care, supervising staff, and ensuring continuity of care for residents in a skilled nursing and rehabilitation facility. Responsibilities include patient assessment, care plan development, communication with the healthcare team, and compliance with documentation standards.
Candidates should have a nursing degree (BSN preferred), a current NY RN license, BLS certification, and ideally three years of clinical experience, particularly in LTAC, Critical Care, or ICU.
