Role SummaryThe RN Case Manager – Pediatrics provides one‑on‑one home‑based nursing care to pediatric patients, primarily ages 0–2. This role focuses on assessments, care planning, education, and coordination — no office time required.
What You’ll Do- Complete initial assessments, recertifications, and manage pediatric caseloads
- Develop and implement individualized pediatric care plans
- Provide clinical care and parent/caregiver education
- Collaborate with an interdisciplinary care team
- Document care accurately and timely
- Active NY RN license (or ability to obtain)
- Pediatric inpatient or pediatric clinical experience
- Strong assessment, communication, and organizational skills
Ability to work independently in a community setting
