Near Newark, NY Are you a dedicated healthcare professional looking to make a meaningful impact while maintaining a healthy work-life balance? We???re seeking a Nurse Practitioner or Physician Assistant to provide primary care services in a skilled nursing facility near Newark, New York.
This role offers the flexibility to design your weekday schedule, professional autonomy, and the support of a collaborative care team. Responsibilities Deliver primary care for both short-term rehabilitation and long-term care patients. Manage chronic diseases, diagnose conditions, and develop individualized care plans. Order, review, and interpret diagnostic tests and lab results. Prescribe medications and monitor effectiveness. Collaborate with physicians and coordinate care with other providers.
Educate patients and families on chronic disease management and healthy lifestyle choices. Build trusted relationships with patients, families, and facility staff to ensure high-quality outcomes. Requirements NP or PA license in New York (active, unrestricted) DEA license with prescriptive authority (active, unrestricted) Minimum 1 year of NP/PA experience in primary care Post-acute care experience (preferred, not required) Benefits Flexible weekday schedule???tailored
