Responsibilities- Conduct home visits.
- Complete all required documentation.
- Communicate with PCP.
- Complete 485 and interim orders.
- Update medication profile.
- Coordinate care with other disciplines.
- Review and complete case communication notes.
- Daily review of follow-up items and incomplete documentation items noted in HCHB.
- Participate in case conferences with respective supervisor.
- $15,000 sign-on bonus after 15 months and full productivity
