LPN Licensed Practical Nurse

Currently we have clients with Assisted Living and Memory Care opportunities, offering FT and PT Day, Evening, and Night openings with rotating weekends.

As an LPN you will support residents by providing hands-on nursing care and administering medications. LPNs may also oversee the work of PCAs on their unit.

• Assures compliance with nursing practices and implements facility policies and procedures.
• Assures compliance with federal, state, and local regulations within the assigned unit.
• Communicates nursing policies and procedures to PCA's, residents, families, and Physicians.
• Conducts daily resident rounds and initiates action to assure resident needs are met.
• Provides direct resident care when needed and provides direction to PCA's.
• Assures that all issues/concerns identified on your shift are addressed or properly communicated to the in-coming nurse or DHS for follow-up and timely resolution.
• Attend staff meetings, in-services, classes, and committee meetings as assigned.
• Assists residents with medications per the Physician's order and accurately records assistance.
• Observes, evaluates, and documents change in resident's condition on the 24-Hour Report and in the Nurses Notes.
• Documents resident care provided and resident's response or lack of response to that care in the medical record.
• Completes required documentation upon admission, discharge, or transfers of a resident to another facility, to assure continuity of care.
• Upon admission, meets with new residents, family members, and the physician to obtain information required to develop a written Plan of Care.
• Reviews and revises residents' Plan of Care with any significant change and then at least annually for Assisted Living Residents and every six months for Memory Care Residents, as required by regulation.
• Provides daily supervision of PCAs and Memory Care Companions.
• May be required to act as the building supervisor in the absence of the Administrator and the Director of Health Services for resolution.

Requires:

• Current NY LPN (Licensed Practical Nurse) license
• Good interpersonal skills that demonstrate an ability to work well with individuals and as a team member
• Respectful of all individuals
• New grads welcome
• Experience in an adult care facility or long-term facility setting preferred.
• Experience working with a dementia population preferred
