Job Description

Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Job Responsibilities
Patient Assessment
  • Assesses the patient’s physiologic health status (including gastrointestinal, cardiovascular, respiratory, renal, and neurological status); determines mobility, sensory deficits, prosthesis use, skin condition, and pain level; documents assessment and communicates findings.
  • Assesses patient’s psychosocial health status, eliciting perceptions of care, coping mechanisms, knowledge level, and cultural/ethnic requirements, and documents psychological status and care plan.
  • Identifies patient outcomes, develops criteria and measurement of outcomes, identifies actual/potential problems, and establishes individualized patient goals.
  • Forms the plan of care and establishes priorities necessary to achieve expected outcomes; coordinates cost‑effective use of supplies, equipment, and medication; documents the plan and collaborates with physicians and other health team members.
Implementation and Ongoing Care
  • Implements the plan of care; maintains vigilance over patient care, ensuring safety precautions such as side rails, call lights, and bedside stands; exercises professional skills related to the plan.
  • Reassesses patients as needed, revises plan; administers prescribed treatments and medication correctly; uses equipment safely and efficiently; provides emotional support and scientific principles for procedures.
  • Makes accurate observations during treatments, reports and records as appropriate, and keeps documentation of patient responses to interventions.
  • Acts rapidly during emergencies, managing self, patients, and other employees; communicates effectively with patients, family, and caretakers.
  • Participates in discharge planning, coordinates referrals, and provides discharge instructions.
Evaluation
  • Evaluates care provided, measuring effectiveness and documenting results; performs variance analysis related to outcome data for performance improvement.
  • Designs, implements, and evaluates systems to improve care in the unit.
Collaboration
  • Collaborates with other care team members in planning and carrying out treatment regimens; provides direction to other members and delegates assignments using clinical judgment.
  • Assists physicians during treatments and diagnostic procedures; keeps the Patient Care Manager or physician updated on changes in condition and treatment.
Decubitus Care
  • Performs grade I–IV decubitus care including preventive skin care measures, simple dressings with aseptic technique, warm & cold compresses, irrigations, and other procedures related to skin care.
Medication Administration
  • Correctly identifies medication action, dosage, side effects, and patient implications.
  • Meets standard on medication administration examination.
  • Prepares local solutions, I.V. drips, and administers and documents medication correctly.
  • Educates patients and significant others on drug‑food interactions; provides patient education and participates in family education.
  • Communicates assessment data to care team members and relevant agencies; evaluates teaching effectiveness through patient/family feedback.
Physical Demands

Constant – standing, walking, stooping, bending, stretching, lifting, pulling, pushing, and turning.

Frequent – lifts objects from 10 lb to 150 lb; pushes objects up to 200 lb.

Occasional – sitting; lifts with assistance objects greater than 150 lb and carries objects weighing up to 50 lb.

ADA Essential Functions
  • Job

    Qualification:

    Graduate from an accredited School of Nursing;
    Bachelor’s Degree in Nursing preferred; must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010, and obtain a BSN within five years.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • BCLS, ACLS, preferred.
