PositionAmbulatory Care Nurse
Responsibilities- Support the coordination, integration, communication, implementation and evaluation of nursing practice and clinical policies and procedures.
- Participate in site and organizational quality and performance improvement activities.
- Collect patient healthcare data systematically using appropriate assessment techniques and instruments.
- Collaborate with licensed providers to develop patient-centered plans of care that prescribe interventions to attain expected outcomes.
- Provide comprehensive care including administration of treatments and medications, wound care, health teaching and health counseling, and health promotion.
- Perform physical examinations and diagnostic tests; collect patient health history; document information in the electronic health record.
- Provide information, counsel, and education to patients and families regarding test results and treatment plans.
- Administer medications (including vaccination, IV therapy, oxygen administration) and perform triage for clinical concerns.
- Assess and monitor patient responses to treatments and anesthesia; interpret patient information and determine appropriate treatment plans in collaboration with supervisors and providers.
- Direct, supervise and delegate to licensed practical nurses, medical assistants and technicians to support patient flow and efficiency.
- Coordinate and participate in clinical standardization and compliance efforts, including Patient Safety initiatives and quality metrics (e.g., MIPS, Care Gap Management).
- Support coordination of on-site activities to provide patient care in a healing, humane environment and collaborate with interdisciplinary team members.
- Serve as a resource for nursing staff and providers; participate in evidence-based practice and clinical inquiry.
- Engage in self-assessment and professional development; maintain current nursing knowledge and competence; mentor peers; report professional misconduct as appropriate.
- Execute medical regimens prescribed by licensed providers and perform assessments, case-finding, care coordination, and direct care services as part of patient plans.
- Perform procedures and manage care such as wound care, device management, and administration of therapies as per scope of practice.
- Maintain and manage IV lines and monitor for complications; administer critical medications as authorized.
- Manage in-baskets, document and report critical lab values, and relay results to patients as directed by providers.
- Perform triage for patient concerns via in-person or phone encounters with appropriate escalation.
- Coordinate care for defined populations with multi-disciplinary plans.
- Identify deviations from health and safety; advocate for appropriate actions and safe environments.
- Collect and analyze patient data to determine health status and nursing diagnoses; educate and empower patients and families.
- Develop patient-centered care plans and educate patients to promote healthy adjustment to changes in health patterns.
- Uphold the NYULH mission and values, promote excellence in patient experience, and collaborate with management to ensure a positive experience.
- Greet patients professionally and communicate care steps clearly; collaborate to resolve inquiries promptly.
- Escalate issues to leadership as appropriate and contribute ideas to improve patient experience and access.
- Partner with access and billing teams to support a positive patient experience.
Current Registered Professional Nurse in New York. If hired after 9/1/25:
Baccalaureate degree in Nursing required;
Associate Degree in Nursing may be approved contingent upon Nursing Leadership approval, with a minimum of two years nursing experience in applicable area and enrollment in a baccalaureate nursing program to be completed within 2 years of hire. Basic Life Support Certification through the American Heart Association.
Registered Nurse License-NYS;
Basic Life Support Certification
ACLS certification. Membership in professional organizations. Cardiology experience.
Qualified candidates must be able to effectively communicate with all levels of the organization.
