Location: New York
Responsibilities
- Deliver comprehensive outpatient care, including routine and acute visits, wellness assessments, and health education.
- Collaborate with a multidisciplinary team and coordinate with specialists and community resources.
- Lead population health initiatives and participate in outreach and home visits as needed.
- Build trusting patient relationships through longitudinal, holistic care.
Master’s degree in Nursing, national certification (Family or Adult‑Gerontology NP), active state license, and DEA registration. Preferred experience in primary care and understanding of HCC/ICD‑10 coding.
