Position:
Registered Nurse (RN) licensed in the state of NY.

Location:

Queens, NY. Prior experience in Pediatrics is required. Valid NY RN license is required.

Pay: $50 per visit to $125 per visit. SOC: $100 per visit with an extra $25 bonus if notes are completed within 24 hours. Follow-up/Revisit: $50 per visit with an extra $25 bonus if notes are completed within 24 hours.

The Pediatric Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education/experience.

• Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status and needs change.
• Develops the plan of care with patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient/family as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient.
• Knowledge of high-tech nursing skills as applicable, including catheter care, ostomy care, tubing care, infusion services, G-tubes, and wound therapy.
• Working knowledge of infection control procedures and safety precautions.
• Knowledge of the nurse’s scope of practice and ability to apply professional theory, practice and procedure.
• Observe ethical and legal guidelines for safeguarding confidentiality of patients.
• Assess patient needs and physical status each skilled visit and re-evaluate through health data assessment, response to therapy, and updated physician orders.
• Inform physician of any changes in patient condition; obtain and clarify orders for Plan of Treatment revisions.
• Prepare clinical notes and update the primary physician as needed and at least every sixty days. Complete and submit documentation per policy and procedures.
• Communicate with the physician regarding patient needs and reports; obtain physician orders as required.
• Coordinate care with other health professionals.
• Participate in on-call duties as defined by the on-call policy.
• Ensure arrangements for equipment and other necessary supplies and services.
• Instruct, supervise and evaluate home health aides and personal care aides every 14 days or as stipulated by law and regulation.
• Instruct, supervise and evaluate LPN/LVN care at least every 30 days.
• Exercise professional management and coordinate care among home, community providers and pediatric specialists.

• Bachelor's degree in Nursing from an accredited college/university.
• Current active and unrestricted nursing license.
• Pediatric care experience in the acute and/or home care environment.
• Valid driver’s license.
