We are seeking a Psychiatric Nurse Practitioner to provide on-site behavioral health services within skilled nursing and extended care facilities. This position provides a highly structured weekday schedule, strong clinical autonomy, and the ability to manage a consistent adult and geriatric patient panel in a non-hospital setting.

• Current DEA registration or ability to obtain
• Strong clinical assessment and diagnostic skills
• Excellent communication and interpersonal abilities
• Ability to work independently while remaining team-oriented

• Conduct comprehensive psychiatric evaluations for new admissions and referred residents
• Provide routine follow-up visits and ongoing medication management
• Develop individualized treatment plans for mood disorders, anxiety disorders, psychotic disorders, and neurocognitive conditions
• Prescribe, monitor, and adjust psychotropic medications in accordance with evidence-based standards
• Address behavioral disturbances associated with dementia and other cognitive conditions
• Collaborate with nursing leadership, primary care providers, and social services staff
• Participate in care plan meetings and interdisciplinary discussions
• Educate patients and family members regarding diagnoses and treatment strategies
• Maintain timely, accurate, and compliant documentation in the electronic medical record
• Ensure services meet all federal and state regulatory requirements

• Experience in long-term care, skilled nursing, assisted living, or rehabilitation settings
• Background in geriatric psychiatry or chronic mental health management
• Prior experience as a Psychiatric NP in institutional settings
