- Coordinate cost‑efficient, reimbursable patient care under the insurance plan and physician approval.
- Review insurance information, verify eligibility, and ensure services align with the care plan.
- Oversee and ensure compliance in clinical documentation, care coordination, and billing processes.
- Participate in QAPI activities and chart audits to uphold high‑quality care standards.
- Supervise and support pediatric nurses through joint visits, evaluations, and development programs.
- Communicate effectively with the RN Case Manager and interdisciplinary team to optimize patient care.
- Facilitate patient discharge planning and ensure thorough documentation of all care coordination activities.
- Licensed and registered as a Registered Professional Nurse in New York State.
- BLS certification required.
- 3+ years of experience in a home care/CHHA environment, with supervisory experience preferred.
- Prior experience with pediatric care and full vaccination for COVID‑19 required.
- Reliable transportation within Brooklyn, Queens, and Manhattan, and the ability to work holidays and weekends on a rotating schedule.
